Privacy Policy
Draft for review
This policy describes how the website was actually built, but it has not been reviewed by an attorney. Have counsel licensed in Connecticut review it before launch, and set the effective date above.
This policy covers this website. Health information you share as a patient of the practice is covered separately and more strictly by our Notice of Privacy Practices under HIPAA.
What this website collects
Two things, and only two.
First, whatever you type into the appointment request form: your name, phone number, email address, the service you are interested in, whether you are a new patient or a referring provider, and anything you choose to write in the optional message box. That form asks you not to include symptoms, diagnoses, or medications, because a web form is not a secure channel for clinical detail.
Second, ordinary web server logs. Like nearly every website, the server records the IP address of visitors, the pages requested, and the time of the request. These are used to keep the site running and to limit abuse of the request form.
- No advertising or analytics trackers are loaded on this site.
- No cookies are set by this website.
- No fonts, scripts, images, or stylesheets are loaded from any third-party domain, so no other company sees your visit.
- No social media pixels, no session recording, no heat mapping.
That is unusual, and it is deliberate. Visiting a behavioral health website should not follow you around the internet afterward.
How that information is used
Information from the request form is used to contact you about care at New Horizon Behavioral Health, to verify insurance benefits when you ask us to, and to schedule an appointment. That is the whole list.
- We do not sell your information.
- We do not share it with advertisers, data brokers, or marketing companies.
- We do not add you to a mailing list because you asked for an appointment.
Server logs are used for security, troubleshooting, and rate limiting the request form so it cannot be abused.
Where it goes and how long we keep it
Requests submitted through the form are delivered by email to the practice and stored on the web server so a request is not lost if email delivery fails. Once a request becomes an appointment, the relevant information moves into your medical record, where it is governed by HIPAA and by our Notice of Privacy Practices rather than by this policy.
Request records that never become appointments are kept only as long as they are useful for following up, and are then deleted.
Who else is involved
The website is hosted by a hosting provider, which necessarily processes the traffic to the site, and email sent by the request form passes through a mail provider. Those companies act as service providers, not as recipients of your information for their own purposes.
If you follow a link from this site to another website, such as the FDA prescribing information linked from the Spravato page, that site has its own privacy practices and this policy stops at our boundary.
Security
The request form checks that submissions come from this website, limits how many can be sent from one connection, and strips characters that could be used to tamper with email. The site loads no third-party code, which removes an entire category of risk.
No transmission over the internet is perfectly secure, which is exactly why the form asks you to keep clinical detail out of it and save it for your appointment or a phone call.
Your choices
You can use the phone instead. Nothing on this site requires you to submit a form, and calling the office at (860) 838-8799 reaches the same people.
You can ask us to delete a request you sent through the website. Email info@newhorizonbehavioralhealth.com or call the office. If you are already a patient, your rights over your medical record are broader and are described in our Notice of Privacy Practices.
Who this site is for
This practice provides care to adults, 18 and over. This website is for adults, and we do not knowingly collect information from anyone under 18 through it.
Changes to this policy
If this policy changes, the revised version is posted here with a new effective date. Material changes to how we handle information you already submitted will be communicated directly rather than by quietly editing this page.
