Terms of Service
Draft for review
Standard terms adapted for a Connecticut outpatient behavioral health practice, but not reviewed by an attorney. Have counsel review before launch, particularly the liability and governing law sections.
These terms govern your use of this website. They do not govern the care you receive as a patient, which is set out in the documents you sign at intake.
Agreeing to these terms
By using this website you agree to these terms. If you do not agree with them, please do not use the site. Call the office instead at (860) 838-8799, which is always an option here.
This website is not medical advice
Everything on this site is general education. The pages about therapy, medication management, and Spravato describe treatments in general terms so that a conversation with a clinician starts from a better place. They are not a diagnosis, not a treatment recommendation, and not a substitute for an evaluation.
Nothing here should be used to start, stop, or change a medication. Do that with your prescriber.
Using this site does not make you a patient
Reading these pages, sending the request form, or emailing the office does not create a clinician and patient relationship. That relationship begins when you are evaluated and accepted for care, and the documents you sign at that point govern it.
Emergencies
This website is not monitored around the clock and must never be used in an emergency. If you are in crisis or concerned about your safety, call or text 988 to reach the Suicide and Crisis Lifeline, call 911 if there is immediate danger, or go to your nearest emergency department. Connecticut residents can also call 211 for crisis services.
Appointment requests
Submitting the request form is a request, not a booking. An appointment exists only once the office has contacted you and confirmed a time. Requests are reviewed during office hours.
Please keep clinical detail out of the form. Ordinary email and web forms are not secure channels for health information, which is why the form asks for scheduling information only.
Acceptable use
You agree not to use this site to send unlawful, abusive, or deliberately false information, not to attempt to gain unauthorized access to the site or its underlying systems, not to interfere with its operation, and not to submit automated or bulk requests through the form. The request form is rate limited and abusive traffic is blocked.
Content and ownership
The text, layout, code, and graphics on this site were created for New Horizon Behavioral Health and belong to the practice. You are welcome to read, print, and share pages for personal use or to give to a patient. Republishing the content as your own, or using it to train a commercial product, is not permitted without written permission.
Spravato is a registered trademark of its manufacturer and is used here only to identify the medication.
Links to other sites
Some pages link to outside resources, such as the FDA prescribing information for Spravato or the National Institute of Mental Health. Those sites are not under our control, and we are not responsible for their content or their privacy practices. They are linked because a claim on our page should be checkable against its source.
Disclaimers
We work to keep this site accurate and current, particularly the clinical information, but we cannot promise that every page is free of error or that the site will always be available. Medical guidance changes, and a page reviewed last quarter may not reflect a change made last week. The site is provided as it is, without warranties of any kind to the extent the law allows.
Limitation of liability
To the fullest extent permitted by Connecticut law, New Horizon Behavioral Health is not liable for indirect, incidental, or consequential damages arising from your use of this website. Nothing in these terms limits liability for the care we actually provide to patients, which is governed by law and by professional standards, not by a website disclaimer.
Governing law
These terms are governed by the laws of the State of Connecticut, without regard to its conflict of law rules. Any dispute arising from the use of this website belongs in the state or federal courts located in Connecticut.
Changes
We may update these terms. The current version is always posted here with its effective date, and continuing to use the site after a change means you accept the updated terms.
